From a medical perspective (weak human bio class memory), there are two types of deaths. One, the brain death, and the second, the cardiac death. And in these terms, if the brain isn't dead, but the heart might be dead, the patient is still not announced dead. This varies legally in every different countries, but in most Western countries, that explanation is implemented. As for your concern about us having senses while our heart is with someone else, despite our physical inabilities due to death, than no, we don't feel alive, as a heart functions as part of a system, like any other organ, and because our systems have ceased to function, for whatever reason it may be, then our heart too, is unable to function. Islamically speaking, our ties with this world are cut off, and that is as simple as the explanation can get. We no longer (not in the case of a shaheed) have a physical body - jasad - but our ruh lives and we are given the senses to experience the interrogation in the grave, the punishments, and the rewards.

Although this requires a detailed answer and explanation of many concepts (time restrictions on my end, I must head to bed), the simplest answer (of course not satisfying) would be that, yes, it is permissible to donate organs, so long as the donation will not lead to the death of the donor. There are many conditions and circumstances one must understand before considering organ donation. For example, there is the donation of organs on which one's life depends, and there is the donation of organs which life does not depend on and because of such conditions, the scholarly opinion changes in each case.

you cannot give your heart to someone....

If you are alive you can donate a kidney because your body can work with one kidney

No you dont because your ruh is not stuck to your organs. Otherwise when you die all your organs are not functioning but the ruh is intact...

No because the heart is not the brain!

All your emotions and way of thinking is in the brain. The heart is connected to your brain, so if the heart is in another persons body it will be connected to his brain not the dead person
